How to choose medical practice software in South Africa

Published by GreenNotes. We make one of the products this guide discusses, so we have an interest in your choice. The checklist is written to be applied to any vendor, including us. It does not rank products, and where we describe GreenNotes we say what it does not do as well as what it does.

No single product is the best medical software for every South African practice. The right choice depends on how you document, how you bill, who does the admin and how much you depend on medical-aid claims. Start by deciding which kind of software you need, then test a short list against the same checklist, with your own consultations and your own billing.

Start with the category, not the brand

Practice software in South Africa falls into four broad categories. Products inside a category differ, but the category decides how much retyping, how many logins and how many vendors your practice will live with.

CategoryWhat it doesOften suitsWatch for
Standalone AI scribeRecords a consultation and drafts a note you copy into your records system.Clinicians happy with their current records and billing who only want faster notes.A copy-paste step after every consult; a separate vendor holding consultation audio; pricing in US dollars or per minute.
Clinical system (EMR)Patient records, notes, letters, scheduling, intake and consent; sometimes AI notes.Practices that bill privately or through a bureau using its own software. How invoices get their details: exports, re-keying or an integration.
Billing systemInvoices, medical-aid claims, remittances, statements and debtors.Practices and bureaus where billing volume matters more than clinical tooling.Clinical notes living elsewhere; codes and dates typed twice.
Connected clinical-and-billing platformClinical record and billing on one patient record, often as separate products.Practices that want a consultation to become an invoice and a claim without retyping. What is actually included in each subscription, and which steps are still manual.

The checklist

Put the same questions to every vendor on your short list, ask for the answers in writing, and test the ones that matter most during a trial. “Not stated” is a different answer from “no”: if a vendor can’t tell you, mark it unknown and ask again.

1. Clinical record depth and specialty fit

  • Does it hold the record your discipline needs: notes, letters, body diagrams, growth charts, forms, consent?
  • Can you configure fields, headings and letterheads, or are you working around a template?
  • Test: rebuild a typical patient file from your current system and see what is missing.

2. AI notes, letters and review

  • Which languages can you consult in, and which languages can the note be written in? Test Afrikaans or isiXhosa if you consult in them.
  • How is the draft reviewed and edited, and is the clinician clearly the author?
  • Are there AI credits, monthly caps, per-minute charges or limits on recording length?
  • Test: record a representative week with consent, count the edits per note and time your review.

3. Billing, claims and account follow-up

  • How does an invoice get its patient, provider, date and diagnosis codes? Who creates it?
  • Does it check claims before sending? How are rejections explained and corrected?
  • Are electronic remittances matched to claims? Does matching record the payment, or is that separate?
  • Does it process payments, or only record them? How are statements and overdue accounts handled?
  • If you use a bureau, how does the bureau get access, at what cost, and under whose permissions?

4. Mobile and web

  • Which features are on the phone, which on the computer, and which on both? Ask for a list rather than “it works everywhere”.
  • Are the mobile apps native, and are they on both iOS and Android?

5. What actually works offline

  • Can you record without signal and upload later? Can you open records that were cached?
  • Does AI processing need a connection? (For almost every product, it does.)
  • Test: switch on flight mode mid-consult and see what survives.

6. South African workflows and local support

  • Medical-aid claims, cover checks, ICD-10 and tariff coding, practice numbers, workers’ compensation forms.
  • Rand pricing, support in South African hours, and the channels your practice actually uses.

7. Data handling, access and your responsibilities

  • Where is the system of record hosted? Where does AI processing happen?
  • How is clinical data encrypted? What is logged: changes only, or reads too?
  • Can you set permissions per person and per feature? Is two-factor sign-in available?
  • Is there a published operator agreement and sub-processor list? Remember that the practice remains the responsible party under POPIA. Our POPIA guide explains what that means.

8. Total cost

Build the yearly total for your own team. The headline price per user rarely tells the whole story.

Cost lineQuestions to ask
Clinician seatsPer clinician or per practice? Monthly or annual?
Admin and bureau seatsAre receptionists, managers and bureau staff charged?
AI usageCredits, caps, per-minute or per-note charges?
Billing and claimsPer seat, per claim, per cover check? Switch or transaction fees?
Add-onsPrescribing, tariff and price lookups, SMS or email, extra storage?
HardwareDevices, device management, scanners?
Setup and migrationOnboarding, training, data import?
LeavingExport fees, read-only access after cancelling, notice periods?

9. Onboarding, migration and exports

  • Can the vendor import your current records? In which format, at what cost, and what is left behind?
  • How long does claims enrolment take, and who does it?
  • What can you export at any time, and in what format? How long do you keep access after cancelling?

How to run a fair comparison

  1. Choose the category first, then shortlist two or three products in it.
  2. Send every vendor the same checklist and ask for written answers with dates.
  3. Trial with the same consultations and the same invoices in each product.
  4. Score each checklist item on evidence you saw, and mark unknowns as unknown.
  5. Price the full year for your team, including what it costs to leave.

Be wary of comparison tables that every vendor wins on its own website, including ours: check the source and the date of every claim about another product.

Where GreenNotes fits

GreenNotes is a connected clinical-and-billing platform, sold as separate products. Measured against the checklist above:

  • Clinical and AI notes: records consultations on iOS, Android and the web portal; notes in 100+ output languages including English and Afrikaans; clinician review on every note; no AI credits or monthly cap, with limits of 2 hours per recording and 1 hour of audio per letter. See AI clinical notes.
  • Billing and claims: invoices are created by your team from consultations, with patient, provider, date and recorded codes filled in; pre-submission claim checks, AI help with rejected claims and electronic remittances need the Medical Aid Claims add-on and claims enrolment. GreenNotes records payments but does not process them, and does not suggest tariff codes with AI. See from notes to claims.
  • Mobile and web: one shared record. Body maps, growth charts, the document scanner and offline recording are in the mobile apps; billing and claims are in the web portal.
  • Data: system of record in South Africa, AI processing in the EU and US, per-practice encryption of clinical fields, a published Data Processing Agreement, six months of read-only access with a bulk export after cancelling.
  • Cost: Clinical from R960 per clinician per month with free admin seats and a 14-day trial; Scripts is priced separately with a 3-day free trial; Billing, Medical Aid Claims and Live Pricing are priced separately. See pricing.
  • Moving across: a free, fast-turnaround service imports your clinical data, and a billing import regenerates back-dated invoices for outstanding accounts. See switching to GreenNotes.
  • Gaps to weigh: no AI processing offline, no payment processing, and no consolidated dashboard across practices for bureaus.

The full picture, including who GreenNotes suits and when it may not, is on the platform overview. If you are focused on AI scribes specifically, our AI scribe guide goes deeper on accuracy, consent and trials.

Frequently asked questions

  • There is no single best product for every practice. A solo psychologist who bills patients directly, a GP practice that claims from medical aids every day, and a multidisciplinary clinic using a billing bureau need different things. Decide which category of software fits your practice, then test two or three products in it against the same checklist, using your own consultations and your own billing.
